ABSTRACT

Objective: To analyze the morbidity and mortality trends of thyroid cancer in China from 1990 to 2019, explore the causes of the trends, and predict morbidity and mortality in the future. Methods: The morbidity and mortality data of thyroid cancer in China from 1990 to 2019 were collected from the 2019 Global Burden of Disease database. The Joinpoint regression model was used to describe the change trends. Based on the morbidity and mortality data from 2012 to 2019, a grey model GM (1,1) was constructed to predict the trends in the next ten years. The model was tested by the posterior error method and residual test method. Results: In all populations, men and women, the AAPC values of the crude morbidity rates were 4.15% (95%CI: 3.86%-4.44%, P<0.001), 5.98% (95%CI: 5.65%-6.31%, P<0.001) and 3.23% (95%CI: 2.94%-3.53%, P<0.001) respectively, the AAPC values of age-standardized morbidity rates were 2.47% (95%CI: 2.12%-2.83%, P<0.001), 3.98% (95%CI: 3.68%-4.29%, P<0.001), 1.65% (95%CI: 1.38%-1.93%, P<0.001), the AAPC values of crude mortality rates were 2.09% (95%CI: 1.92%-2.25%, P<0.001), 3.68% (95%CI: 3.45%-3.90%, P<0.001), 0.60% (95%CI: 0.50%-0.71%, P<0.001). The age-standardized mortality rates in men showed a fluctuating trend of first decrease (1990-1994), then increase (1994-2012), and then decrease (2012-2019) (AAPC=1.35%, 95%CI: 1.16%-1.53%, P<0.001). The age-standardized mortality rate in women continuously decreased (AAPC=-1.70%, 95%CI: -1.82%- -1.58%, P<0.001). The GM (1,1) models can be used for medium and long-term predictions. The results of the residual test show that the average relative error values of all models are less than 10.00%, the prediction accuracy values are more than 80.00%, and the prediction effects are good. The results of the posterior error method show that all the prediction results are good except the qualified prediction of the age-standardized morbidity rate in men. In 2029, the crude morbidity rates would increase to 3.57/100 000, 2.78/100 000, and 4.40/100 000, respectively, and the age-standardized incidence rates would increase to 2.38/100 000, 1.89/100 000, and 2.88/100 000, respectively, the crude mortality rates would increase to 0.57/100 000, 0.62/100 000 and 0.53/100 000, and the age-standardized mortality rates would decrease to 0.33/100 000, 0.42/100 000 and 0.27/100 000 in all population, men and women in China. Conclusions: The overall, gender- specific age-standardized mortality rates showed downward trends in the last decade or so, and the prediction results showed that it might further decline. However, the crude morbidity rates, age-standardized and crude mortality rates have been on the rise, and the population aging is becoming increasingly serious in China, which requires close attention and targeted prevention and control measures.

ABSTRACT

Objective:To explore the construction and application methods of multicenter bone tumor-specific database.Methods:Experts from multiple centers including Shanghai General Hospital, Shanghai Changzheng Hospital, Zhongshan Hospital, Shanghai Sixth People's Hospital, Ruijin Hospital, Fudan University Shanghai Cancer Center and Shanghai Ninth People's Hospital established a standard dataset for bone tumors through research and discussion. Clinical data will be automatically collected and standardized according to standard fields. A database will be built and a users' interface will be developed to ensure secure data storage, while providing services such as exporting raw data, visualizing statistical analysis, establishing clinical queue research projects, et al. Finally, the bone tumor database will be shared by integrating with the Shenkang's Big Data Platform to achieve multi-center data integration.Results:A standard data set for bone tumors containing 603 fields has been established and published. An automated data collection system for bone tumors has been established, including complete data collection, data collation and visualization functions. The data categories include modules such as patients' electronic case information, laboratory information on blood routine, biochemistry and tumor markers, imaging information, surgery information, pathology information and radiotherapy records. Personal information such as patients' names and ID numbers are desensitized and encrypted and can be exported for further research. From 2015 to 2023, the total number of bone tumor cases collected in the database was 10,789. From 2015 to 2019, 112 cases of the osteosarcoma cohort were retrospectively analyzed for admission, with a statistical 5-year survival rate of 68%.Conclusion:A regional bone tumor specialty big data network and data sharing platform has been established, along with data sharing mechanisms and standards including data standards, security standards, and quality evaluation standards. This provides data and efficient new solutions for the construction of China's bone tumor database, as well as a research and development platform for standardized diagnosis and treatment of bone tumors and new technologies.

ABSTRACT

Digital and intelligent technology is an important driving force to promote the transformation and innovation of medical service mode and improve service quality. In February 2020, Peking Union Medical College Hospital established an intelligent outpatient pre-admission medical history collection system in order to improve the intelligent level of clinical application business system and the medical experience of outpatients.The system consisted of artificial intelligence capability layer, application service layer, capability resource layer and user application layer and hardware technology architecture. By docking with the hospital information system and using mobile terminal App, patients could submit pre-admission medical history information by themselves, and generate structured patient history for the visiting doctors to call. By November 2021, the pre-admission medical history collection system had covered 80% of the departments of the hospital, serving 1.68 million people in total, and the patient filling rate had reached 60%. The duration of patient′s medical history collection and the duration of doctor′s outpatient electronic medical record writing had been continuously shortened, greatly improving the efficiency of outpatient medical services.

Objective The internationalization of modern scientific research and its high degree of complexity have showed the importance of scientific research cooperation.How to promote cooperation more effectively is an important research topic.Methods This article analyzes the research cooperation of Institute of Hematology & Blood Diseases Hospital Chinese Academy of Medical Sciences during the past 12 years.Results Scientific research cooperation is a big trigger for the output of papers,especially high-level output,international and native institutions' cross-cooperation can be used as an important way for expanding scientific research cooperation and provides a reference for decision-making.Conclusions Strengthen the awareness of cooperation,increasing scientific research funding,establishing common and hot research directions and constructing highlevel platform can expand scientific research cooperation.

ABSTRACT

Objective To study the correlation between TCM constitutions and sleep quality in the elderly population in Hunan Province; To provide a basis for improving sleep quality and adjusting the unbalanced constitutions of the elderly. Methods Multi-stage stratified sampling combined with convenient sampling method was used. TCM constitution rating scale, Pittsburgh sleep quality index, and self-designed general demographic, health and lifestyle questionnaire were used to conduct cross-sectional survey of elderly people living in Hunan Province. Results PSQI scores were negatively correlated with the balanced constitution, and positively correlated with unbalanced constitution, and sleep disturbance was closely related to yang deficiency and qi deficiency. The damp heat, phlegm-dampness, blood stasis, yin deficiency and qi deficiency got the highest scores in the five factors of sleep latency, sleep duration, sleep efficiency, sleep disorders and daytime dysfunction respectively. Conclusion Sleep problems of the elderly in Hunan Province are more prominent, and constitutions and sleep quality affect each other. Different TCM constitutions of sleep disorders in the elderly are different, and should be adjusted according to the constitutions.

ABSTRACT

The concept of golden mean (Zhong Yong), i.e. moderation, is the highest principle of moral cultivation in Confucianism. It entails an exclusive methodology in addressing politics, philosophy and everything in the universe. This concept has permeated into all aspects of traditional Chinese culture. As an ancient therapy, tuina is an important part of Chinese medicine and has its characteristic theory and methods. Just like Chinese medicine, tuina has long been greatly influenced by the concept of golden mean. To better promote the development of tuina, it’s necessary to investigate the historical evolution of tuina and the concept of golden mean as well as the influence of ‘golden mean’ idea on tuina manipulation, mechanism and treatment.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the effect of androgen receptor (AR) on IgG protein expression and the proliferation and migration of prostate cancer cells.</p><p><b>METHODS</b>Western blotting was used to detect the expression of AR protein and IgG in androgen-dependent prostate cancer LNCap cells and castration-resistant prostate cancer PC-3 cells. In AR-overexpressing cells (PC-3-AR cells) established by transfecting PC-3 with AR gene (pCDNA3.1) and LNCap cells with small interfering RNA-mediated AR silencing (LNCap-siAR cells) were analyzed for expressions of AR protein and IgG with Western blotting; the expression of IgG mRNA was detected by Q-PCR, and the cell proliferation and migration were assessed with MTT assay and wound healing assay, respectively.</p><p><b>RESULTS</b>Compared with PC-3 cells, LNCap cells expressed a higher level of AR protein and a lower level of IgG (P<0.05). PC-3-AR cells showed attenuated proliferation and migration with a lowered expression of IgG (P<0.01), while LNCap-siAR cells showed enhanced proliferation and migration with increased expression of IgG (P<0.01).</p><p><b>CONCLUSION</b>The expression of AR is inversely correlated with IgG and is associated with the proliferation and migration of prostate cancer cells in vitro.</p>

ABSTRACT

Objective:This article collected, organized, and summarized the medical literatures of cupping therapy from three major Chinese databases including China National Knowledge Infrastructure (CNKI), Chongqing VIP (VIP), and WanFang, with cupping, moving cupping, flash cupping, pricking cupping, or medicinal cupping as the retrieve title or key words, and the retrieve range was from 1953 to July 15, 2015. This article was to give an overall review of cupping therapy from the clinical application and mechanism research based on its application form, indications, and mechanism research progress. The results showed that there are various types of cupping methods; its indications cover multiple subjects such as internal medicine, external medicine, gynecology and pediatrics; and its major action mechanisms include negative pressure suction, increasing pain threshold, promoting dermal blood flow and improving body immunity.

ABSTRACT

Objective To reveal the evolution, research hotspots and research fronts of clinical pharmacy services. Methods Documents related to clinical pharmacy services from 2000 to 2015 in Web of Science Core Collection database are included.Documents are visually analyzed through a number of co-citation maps, which identified the critical node documents in the revolution of clinical pharmacy services research by knowledge visualization tool, CiteSpace. Its functions of clustering and burst terms detection were also applied to analyze the research hotspots and research fronts. Results This paper determined the distribution of documents on time period, journals, countries and research institutions. Critical node documents perfectly showed the evolution pathway of clinical pharmacy services, main cluster groups and burst terms and documents, indicating the research hotspots and research fronts. Conclusion The research hotspots focus on 3 aspects.1 .Practice, including the service model and content of clinical pharmacy services;its interventions in diseases of emergency, organ transplantation, cardiovascular disea-ses and diabetes;surveys on developing clinical pharmacy services;genotype-guided individualized dosing;remote pharmacy serv-ices.2.Evaluation, including the assessment of the effects of pharmacists' interventions, service quality evaluation, and pharmaco-economic evaluation.3.Pharmacists' career development, including Pharmacists' training pattern, qualification, pharmacy residen-cy programs and job satisfaction.

ABSTRACT

<p><b>OBJECTIVE</b>To explore the application of MS-CT and 3D reconstruction in diagnosis and treatment of vascular malformations in head and neck.</p><p><b>METHODS</b>20 cases with vascular malformations in head and neck underwent MS-CT and 3-D reconstruction. Then the treatment was determined based on the results of MSCT scanning. The postoperative results were evaluated.</p><p><b>RESULTS</b>The images of MS-CT showed the edge of vascular malformations partially or completely in 16 cases of venous malformations. The lesion's anatomic site and 3-D position was obtained. The 3-D images also showed the overexpanded supply arteries in 4 case of arteriovenous malformations. 2 case of venous malformations in lip underwent resection and healed completely. 12 cases of venous malformations in buccal and floor of mouth were treated with compartmentalized sclerotherapy with partial lesion involution. 2 case of venous malformations in mouth floor were treated with operation followed by sclerotherpy with partial lesion involution. 4 cases of arteriovenous malformations were treated with Superselective Artery Embolization with partial lesion involution.</p><p><b>CONCLUSION</b>MS-CT and 3D reconstruction can play an important role in diagnosis and treatment of vascular malformations in head and neck.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the prevalence of tooth erosion of Xuzhou city's children.</p><p><b>METHODS</b>The stratified, cluster and random sampling methods were performed. The prevalence of tooth erosion of 1,219 5-year-old children and 786 12-year-old children in Xuzhou city were examined by one qualified dentist. The results of clinical examination were recorded by schedule table. The risk factors of tooth erosion were investigated by questionnaire and analyzed by Logistic regression model.</p><p><b>RESULTS</b>In 5-year-old children group, the prevalence of tooth erosion was 10.91%. In 12-year-old children group, the prevalence of tooth erosion was 22.14%. The ranking of tooth erosion were mostly class 1 and class 2. The tooth erosions of class 3 and above were rare. The odds rations for tooth erosion were: Acidic fruits, 1.120; acidophilous milk, 1.062; sport drinks, 1.159; carbonated drinks, 1.151; fruit juice, 1.187; drinking acidic drinks or acidophilous milk before sleeping, 6.102; gastroesophageal reflux disease, 2.311; vitamin C, 1.565; supply chalybeate, 1.598.</p><p><b>CONCLUSION</b>The prevalence of tooth erosions in Xuzhou is extensive. Oral health education and drink and food guidance should be strengthened. The amount and frequency of intake of acidic food and drink should be reduced to promote oral health.</p>

ABSTRACT

<p><b>OBJECTIVE</b>The aim of this survey was to obtain some information about the prevalence of deciduous tooth caries of 5-year-old children in Sichuan province.</p><p><b>METHODS</b>Based on World Health Organization and national oral health survey standards, dental caries prevalence and treatment needs were investigated randomly in selected children of Sichuan province in 2005. And the differences between this survey result and the dental caries prevalence of 5-year-old children of Sichuan province in 1995 were compared.</p><p><b>RESULTS</b>The prevalence rate of deciduous tooth caries of 5-year-old children in Sichuan province was 58.72%. The mean number of dmft of 5-year-old was 2.77. The filling rate was only 2.32%. The prevalence rate and mean dmft of deciduous tooth caries showed no significant difference between different area and gender; but the children came from city showed significant higher filling rate than those from countryside, so did the female children compared with the male children. Compared these results with the dental caries of 5-year-old children in Sichuan province in 1995, the prevalence rate and mean dmft of deciduous tooth caries in Sichuan province in 2005 had no significant difference, but the filling rate decreased significantly.</p><p><b>CONCLUSION</b>The deciduous tooth caries prevalence rate of 5-year-old children is still relatively high in Sichuan province; there remains a proportion of active untreated decays. The dental health care education to the parents and teachers should be emphasized. In order to get the earlier diagnosis and treatment, regular examination and prevention method should be conducted.</p>

Objective To investigate the variance of cost-effectiveness when treat acute myocardial infaretion of different severe extents with different pattern.Methods Acute myocardial infarction patients were selected from emergency eommand center of Guangzhou from October 2003 to December 2005.These patients wew assigned by the center to First-Class Hospitals at Grade 3 and First-class Hospital at Grade 2,and were followed up after 6 months after post-discharge.Cost in hospital and mortality in hospital were registered.The health of all patients were quantificated using SF-36.According to the assigned hospitals,the patients were divided into single infarction group and complex infarction group.Cost in hospital,mortality in hospital,short-term quality of life were compared between the them.Results Compared with and First-Class Hospital at Grade 2 (101 cases),the single infarction patients in First-Class Hospitals at Grade 3 had higber costs in hospital (P=0.016),better society function,affection role,mental health and health status (P

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the diagnostic value of multi-slice spiral CT (MSCT) three dimensional (3D) reconstruction for maxillofacial diseases.</p><p><b>METHODS</b>Sixty patients with maxillofacial diseases underwent the scanning of MSCT with 3D reconstruction. Among them, 34 patients with maxillofacial fracture, 10 patients with maxillofacial tumors and tumor-like diseases, and 16 patients with congenital deformities. The MSCT scanned with slice thickness of 2 mm. The methods of 3D reconstruction included multi-planar reconstruction (MPR), shaded surface display (SSD), and volume rendering (VR). The results were compared with what was observed during operations.</p><p><b>RESULTS</b>Totally 36 cases of maxillofacial fracture were shown by 2D or 3D imaging and were validated by the observations during operation. The MSCT with 3D reconstruction imaging was significantly superior to 2D axial imaging in maxillofacial fracture. Three dimensional imaging could clearly show the spacial anatomy of facial, fragment displacement, and tracing fracture lines. However, 2D imaging had better effectiveness than 3D imaging in observing deep structure and fine fracture. In maxillofacial tumors and tumor-like diseases, 3D imaging was significantly superior to 2D axial imaging in showing the tumor shape and spacial relationships between tumors and surrounding structures. Two dimensional imaging and MPR imaging were excellent to reveal internal structure and pathological changes of tumors. 2D imaging and MPR imaging also achieved better results in showing tumors extended to soft tissues. In maxillofacial congenital deformities, 3D imaging were superior than 2D imaging.</p><p><b>CONCLUSION</b>3D imaging has an important value in the diagnosis and clinical assessment of maxillofacial fracture, tumor-like diseases, and congenital deformities.</p>

ABSTRACT

<p><b>AIM</b>To study the therapeutic effects of oxyphenamone, a novel inodilator, on myocardial ischemia.</p><p><b>METHODS</b>The cardiac hemodynamic variables in cats with acute myocardial infarction induced by occlusion of the left anterior descending coronary artery (LAD) were recorded with a physiological polygraph and electromagnetic flowmeter. A model of myocardial necrosis induced by subcutaneous injection of isoproterenol was used for evaluating the effects of drugs on myocardial enzymes and morphological change.</p><p><b>RESULTS</b>Intravenous injection of oxyphenamone (2 - 8 mg x kg(-1)) dose-dependently decreased heart rate, mean arterial pressure, vascular resistance and the parameters of myocardial oxygen consumption (tension time index, TTI) in cats with myocardial infarction. It increased myocardial contractile force and cardiac output transiently but showed no influence on the left ventricular pressure and cardiac work. The changes of myocardial morphology, creatine phosphate kinase (CPK), malodialdehyde (MDA) and serum glutamic-oxaloacetic transaminase (GOT) induced by isoproterenol in rats were diminished by intraperitoneal injection of oxyphenamone (4 - 8 mg x kg(-1)).</p><p><b>CONCLUSION</b>By the examination of the cardiac hemodynamics, myocardial enzymes and morphology, it showed that the myocardial damage induced by ischemia or beta-agonist can be antagonized markedly by oxyphenamone, indicating that oxyphenamone may be beneficial for the treatment of myocardial infarction.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the effect of beta irradiation on intimal proliferation and apoptosis in vein grafts.</p><p><b>METHODS</b>Autogenous vein graft model was established in 80 rats by transplanting the internal branch of the jugular vein to the carotid artery by end to end anastomosis. The veins were irradiated by (32)P solution before anastomosis. Two dose schedules were studied: control group (graft, nonirradiated) and radiation group (20 Gy). The grafted veins were harvested at 3, 1, 2 and 4 week respectively after the operation. intimal hyperplasia (IH), smooth muscle cell (SMC), proliferation, p53, bcl-2 and bax were observed pathologically and immunohistochemically. They were analyzed by a computerized system. The presence of apoptotic VSMC was demonstrated by TUNEL method.</p><p><b>RESULT</b>There was a significant decrease in the average intimal thickness at 7, 14 and 28 days (t = 15.694, P < 0.05) in the radiation group. Immunohistochemical analysis of PCNA indicated decreased positive cells in the radiation group compared with the controls at 1 and 2 weeks (t = 60.157, P < 0.01). Apoptosis of VSMC was higher in the radiation group than in the control group at 2 weeks (t = 56.176, P < 0.01). There was no significant difference in expression of P(53) between the two groups, and there was a significant increase in bax/bcl-2 in the radiation group at 2 weeks (t = 9.783, P < 0.05).</p><p><b>CONCLUSION</b>These preliminary results demonstrated that low dose of beta irradiation in the vein graft inhibits SMC proliferation and induces the apoptosis of VSMC in rats.</p>
